What is IMA?
IMA is an organization that brings together people who would benefit from seeing integrative health professionals, but who could normally not afford the cost of their services, with professionals who will treat people for a reduced fee.

How much does an office visit cost?
Initial visits with any practitioner cost $20.
Follow-up appointments cost $10.
All fees must be paid at the time of the visit.
What are the requirements for IMA membership?
Applicants must:
1. Have a current primary care physician;
2. Meet financial requirements based on the size of their household;
Applicants must make less than the maximum amounts listed here.
For a family of 1, you must make $22,980 or less.
For a family of 2, you must make $31,020 or less.
For a family of 3, you must make $39,060 or less.
For a family of 4, you must make $47,100 or less.
For a family of 5, you must make $55,140 or less.
3. Show proof of household income once a year;
Income can be verified through official tax forms (1040, W2, or 1099), 2 recent pay stubs, or other official documentation.
4. Complete and sign the IMA membership application.
A complete application includes both the online or PDF portion of the application and proof of income.
How do I send in my application and proof of income?
You can submit your application online or you can print it out and mail or fax it to us. You must send us your proof of income by either uploading it or by mailing or faxing it in.
What happens after I send everything in?
The staff of IMA will review your application and income verification. If accepted, you will be sent an email explaining how you can find practitioners in your area and schedule appointments. Your membership will be valid for one year.
